Responsible body: District civil registry offices
Guide to this service
A surname, first name or patronymic is changed by the civil registry office of the district or city where you are registered, on form CR-9. A change at marriage is entered at the registration of the marriage; a return to the traditional form of a surname (…uulu, …kyzy) is free of state duty once.
Who can apply
- Anyone aged 18 or over, for himself or herself. A child's name is changed on the application of both parents; a child aged 10 or over must agree in writing.
- At marriage no separate application is needed; at divorce the pre-marital surname can be restored at the registration of the divorce or later by application.
What to bring
- Form CR-9
- Your identity card and birth certificate
- Your marriage or divorce certificate, if you have one
- Birth certificates of your children under 16
- For a return to the traditional form: a document showing the father's name, if the registry asks for it
